Waiver
I understand that I am volunteering for the 2026 Macaulay Honors College Willowbrook Park BioBlitz in or around Willowbrook Park on August 29 2026 and/or August 30 2026 (the “Program”), and that I will not be paid for my services. The Program begins with with check-in for shuttle bus service or at the Willowbrook Park host site, and concludes upon release from my survey team at the end of my shift at or near the host site, or from the shuttle bus at or near the shuttle stop.
I wish to participate in the Program and understand that such participation involves risks and hazards not found in study at the College, including risks involved in traveling to, from and within the Program, risk of damage to personal property or serious injury, including permanent disability and death, which might result not only from my own actions, inactions or negligence, but the actions, inactions or negligence of others, the condition of the premises or of any equipment used, water hazards, and the effects of weather, including heat, humidity, cold and/or precipitation and I hereby voluntarily elect to participate in the Program. I acknowledge, accept, and assume all such risks, whether or not foreseeable and whether or not caused by the negligent or intentional acts or omissions of others.
I understand that I am responsible for my own behavior, and will only perform work that I feel comfortable and safe doing. I further agree that I will become informed of, and will abide by, all laws and standards relevant to the Program and will comply with the University and College Code of Conduct and University Henderson Rules.
I understand that in order to participate in the Program, I must wear closed-toe shoes and clothing that covers my legs. I understand that if I am not dressed properly on the day of the Program, I may not be permitted to participate. I understand that I am responsible for personal protection from natural elements including but not limited to sun, rain, poison ivy, and insects.
I hereby release and agree to indemnify and hold harmless Macaulay Honors College, The City University of New York, the Greenbelt Conservancy Inc., Willowbrook Park, the New York City Department of Parks & Recreation, and the City and State of New York (collectively, the “Released Parties”) from any liability, claims and causes of action arising out of or related to any loss, damage or injury to me or my property that I may sustain in connection with or arising out of my participation in the Program. If I am injured while performing my assigned duties as a volunteer in connection with the Program, I agree to notify my supervisor/team leader immediately, or as soon as possible. I hereby release and forever discharge the Released Parties from any claim whatsoever that arises or may arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my participation in the Program.
I have no known physical or health-related reasons or problems that preclude or restrict my participation in the Program or I have disclosed to the College any physical, mental, and emotional conditions or problems, permanent or temporary, including special dietary and medication needs, or the need for visual, auditory, or mobility aids, that might impair my ability to participate in the Program, and I hereby release each of the Released Parties from any and all claims, injuries, damages, or loss (including death) arising out of my failure to disclose such conditions or problems.
I hereby authorize and give consent to the Greenbelt Conservancy Inc., Willowbrook Park, New York City Department of Parks & Recreation, Macaulay Honors College and City University of New York to take photographs and videos of me while participating as a volunteer in the Program; to copyright or publish any and all photographs or videos in which I appear while enrolled as a volunteer in the Program; and to use or cause to be used, these photographs, digital videos, or videotapes for any non-commercial purposes, including all exhibitions, public displays, art and non-commercial advertising purposes without limitations or reservation.
I am at least eighteen (18) years old, wish to participate in the Program and have read and completed this waiver and release and am signing it voluntarily.
